Student-Athletes must have a valid physical exam and an updated
Health History form submitted to participate in any sport. The physical
exam must be approved by our school doctor prior to participation in any
tryouts, practices, scrimmages, and/or games.No On-line Registration and No forms = No participationNo excuses!!!All forms must be submitted to the Nurse's Office.
IMPORTANT INFORMATION ABOUT PHYSICAL EXAMSNew Jersey requires that all students must pass a licensed physician's or licensed nurse practitioners physical examination PRIOR to participation in any sport. The school physical form is the ONLY form acceptable and must be reviewed and approved by our school physician. The exam shall be conducted within 365 days prior to the first practice session.Physical exams completed by a private physician MUST be submitted to the Athletics Director's Office at least 2-3 weeks prior to the start of the sports season you wish to participate in. A new seasonal update form must be submitted each athletic season (fall, winter and spring). Physicals will be conducted by our school physician on the dates listed below. Physicals are by appointment ONLY and students MUST sign up in advance in the Nurse's Office. All forms must be completed and signed prior to the physical.PHYSICALS WILL BE HELD AT MEDFORD ON THE DATES BELOW:November 6, 2017November 28, 2017December 12, 2017Students must make an appointment with the school nurse prior to the physical date. Students should ensure all athletic forms are completed and brought to their appointment.You must have a physical on file to tryout or participate in athletics. If you are already a BCIT student, check with the school nurse on the status of your current physical. They expire after one year.***Please be reminded student-athletes must register online for athletics each season and submit the other medical required forms to the nurse to receive final clearance. Important Information About Concussions
The NJSIAA has issued an official policy statement in which it addresses concussion identification, management and return to play. BCIT's policy mirrors NJSIAA's policy and there are strict guidelines for suspected or actual concussions sustained by athletes. There is also an additional form that will be collected from all athletes with their physical paperwork. This form specifically outlines the steps that will be taken in such incidents. Below are several links with pertinent concussion information. We encourage parents and athletes to be familiar with this information.
