Under “Forms”, fill out 2023-24 Spring Sports Participation, Select the Sport/Activity
Under “Forms”, fill out Athletics Health History Update (Spring 2024)
VALID PHYSICAL EXAM ON FILE WITH THE NURSES OFFICE
If you choose to use the school physician, the Physical Exam Form must be completed, signed/dated
and submitted by February 15 - once a completed form is submitted, you will receive an appointment to see the School Physician.
OUR SCHOOL PHYSICIAN WILL BE HERE ON FEBRUARY 22
If you choose NOT to use the school physician, the Physical Exam Form must be completed by a private physician
and MUST be submitted to the Nurse's Office by February 26 at 8:30am if you wish to participate.
If your student has asthma or any allergy requiring epinephrine, you must complete an ASTHMA ACTION PLAN AND/OR AN ANAPHYLAXIS PLAN.
Both are located on the left-hand side menu.
*All students must be accounted for every day for safety reasons. Therefore, if your child is absent, or late to school, be sure to call the MAIN OFFICE.

Doctors Note Policy:
All doctors' notes must be turned into the main office for processing. The Main Office is in charge of attendance adjustments and will excuse an absence for an appointment. Please do not give ANY doctor's notes to the nurse. The main office will process them, and fax them to the nurse who will disseminate them to the proper channels. My name is Miss.Sgouros and I am your school nurse. I have a Bachelor's in Health Science from Stony Brook University, and a Bachelor's in Nursing from Thomas Jefferson University. I have a Belgian Malinois named Zellie who I take to the dog park in my free time. My likes - Ocean City New Jersey, the Philadelphia Phillies, Philadelphia Eagles, food trucks, and spending time with friends and family. I love being a nurse and being able to work with the families in the BCIT community to keep the students healthy, safe and ready to learn!
