SPORTSIf you choose NOT to use the school physician, the Physical Exam Form must be completed by a private physician and MUST be submitted to the Nurse's Office by February 26 at 8:30 am if you wish to participate.

ALL Student-Athletes MUST be registered through the Genesis Parent Portal to participate.
This includes filling out the Athletic Physical Update Release.
- Please make sure that all physicians/providers perform a vision screening with the students' Physical Examination.
- Please make sure the doctor signs the bottom portion of the Completed Cardiac Assessment Professional Development Module. Remind the Physicians/Providers that the date of the Cardiac Module WILL BE DIFFERENT from that of the physical exam date. We require the module completion date, in order to officially clear the athletes with the state.
- IMPORTANT: New Jersey requires that all students must pass a licensed physician or licensed nurse practitioner's physical examination prior to participation in any sport. The school physician form is the only acceptable form and must be reviewed and approved by our school physician. The exam shall be conducted within 365 days prior to the first practice session.
- Physical exams completed by a private physician MUST be submitted to the Nurse's Office at least 2 weeks prior to the start of the sports season you wish to participate in.

Medford Campus Rules and Procedures for Spectators at Inside Athletic Events
- All spectators must remain only in authorized areas during games to ensure safety. Spectators are not permitted to enter restricted areas where signs are posted, gated off areas, or other areas designated by the Site Supervisor and/or Athletic Director.
- Spectators are not permitted to sit on the team bench during games.
- Spectators are not allowed in the locker rooms.
- All spectators must supervise their young children to ensure safety. Children should not climb on the outside or under the bleachers.
- Spectators must be respectful and appropriate towards the players, coaches, and officials at all times.
- Practices are closed to the public. Only team players and school personnel are allowed in the building during practices. Parents/guardians should wait in the parking lot during practices if they are picking up their child or children.
- Spectators are not permitted to eat or drink in the gym.
- Failure to follow these rules may result in having to leave the school grounds.
Medford Campus Rules and Procedures for Spectators at Outside Athletic Events
- Spectators are not permitted to smoke on school property. Smoking includes but is not limited to the following: cigarettes, cigars, pipes, or devices containing tobacco. Electronic smoking devices are also prohibited.
- Pets/Animals are not permitted on school grounds during athletic activities.
- All spectators must remain only in authorized areas during games to ensure safety. No spectator is allowed to enter restricted areas where signs are posted, field paint is used as a barrier on the ground, gated off areas, or other areas designated by the Site Supervisor and/or Athletic Director.
- All spectators must supervise their young children to ensure safety. Children should not climb on the outside or under the bleachers.
- Spectators are not allowed to sit on the team bench during games.
- Spectators must be respectful and appropriate towards the players, coaches, and officials at all times.

- The number of spectators permitted to attend an outside athletic event will align with Governor Murphy's Executive Order in place at the time of the contest.
