  • All students must be accounted for every day for safety reasons. Therefore, if your child is absent, or late to school, be sure to call the main office.

Doctors Note Policy:

  • All doctors' notes must be turned into the main office for processing. The Main Office is in charge of attendance adjustments and will excuse an absence for an appointment. Please do not give doctor's notes to the nurse.

  • The main office will process them, and fax them to Mrs. Fowler who will disseminate them to the proper channels. Doctor's Notes sent solely to the nurse's office cannot guarantee a delivery to the correct destination.

Student Health and Safety: 

  • It is important that I am made aware of all students health conditions that may require interventions so that your child is safe during school hours. All student records and medical information is strictly confidential. 

To administer over-the-counter & prescriptive medication we must have your physician's order and parent permission. Conditions may include, but are not limited to: 

  • Asthma

  • Diabetes

  • ADHD, ADD

  • Seizures

  • Cardiac conditions

  • Orthopedic conditions


Food Allergy and Anaphylaxis Plan

Life-threatening allergy to peanuts, treenuts, any other foods, bees, or latex? 

Have your student's healthcare provider complete this form which also need to be signed by the parent/guardian.

Submit completed forms to the nurse. 

Asthma Treatment Plan

Asthma Treatment Plan to be completed by healthcare provider AND by parent.

Submit the completed form to the nurse. 

Students may carry rescue inhaler during the school day and for all after school sports or activities and for school trips, so it is available to the student when needed.

Medication Dispensing Form

This form is to be completed by the healthcare provider and signed by the parent/guardian. 

ALL medications need to be administered by the school nurse.  Students may not self-carry/self-administer any medications, not even Tylenol, Tums, ibuprofen, etc. 

Completed forms are to be submitted to the school nurse.  Parents/guardians need to bring the medication to school (medications may not be brought to school by the student).

Seizure Action Plan - Please have your doctor fill out the seizure action plan and return it to the school nurse.

Diabetes

The Diabetes Medical Management Plan needs to be completed by the student's healthcare provider and parent/guardian.  Submit completed form to the school nurse. Students with diabetes may self-carry all their supplies on their person.

How do I sign up?

  1. Complete the athletic registration - go to the Genesis Parent Portal and sign up for a sports/club.

  2. Have your doctor and your parent complete the preparticipation physical form - (see above) and turn it into the nurse. You only need to turn in the medical eligibility form into the nurse, not the entire physical.

  3. If you have asthma - you must ALSO complete an asthma treatment plan ( see the asthma treatment plan above).

  4. If your physical is greater than 90 days old, you must also complete a health history update form (See the Genesis Parent Portal).

When are my forms due?

Fall - August 4, 2025

Winter - November 11, 2025

Spring - February 23, 2026

How long does it take to get cleared?

It can take 24-48 hours, Monday through Friday, not counting weekends.
